The jar lid does not budge and you hand it to someone else. Carrying groceries in one trip has quietly become two trips. You noticed your handshake feels different, and picking up a grandchild involves a calculation you never used to run.

None of that came up at your last physical. Blood was drawn, your pressure was taken, your cholesterol got discussed, and nobody asked you to squeeze anything or timed you walking down the hallway. You left with a folder of numbers describing your chemistry and nothing at all describing your capacity.

The distinction turns out to be enormous. Here is what the research says about that omission. Some of the strongest predictors of how the next twenty years will go for you are simple physical measurements that take under a minute and cost essentially nothing, and one of them outperformed a vital sign that every clinic in the country checks at every visit. In my practice caring for people in The Villages, this is the gap I find most striking in standard care. Let me show you the studies, because the numbers behind them are more convincing than any argument I could make on my own.

The Handgrip Finding That Surprised Everyone

Researchers running the Prospective Urban Rural Epidemiology study measured handgrip strength in nearly 140,000 adults across 17 countries, using a simple device called a dynamometer, then followed those people for roughly four years. The results appeared in The Lancet.

Grip strength predicted death. Each 5 kilogram reduction in grip strength was associated with the following increases in risk.

17%higher risk of cardiovascular death
7%higher risk of heart attack
9%higher risk of stroke

Then came the line that made this study famous.

Grip strength was a stronger predictor of all-cause and cardiovascular mortality than systolic blood pressure. A thirty second squeeze test outperformed the number your clinic checks at every single visit.

The associations held after adjusting for age, physical activity, education, and health status, and they held consistently across countries at very different income levels. This was not simply sick people having weak hands.

Understanding why requires seeing what grip actually reflects. Your hand strength serves as a reasonable proxy for total skeletal muscle mass and for the integrity of the neuromuscular system driving it. Muscle is where most of your glucose gets stored, where your metabolic reserve lives, and what determines whether you can get yourself off the floor at eighty. A grip measurement samples all of that at once.

Walking Speed Is Called the Sixth Vital Sign

The second measurement is even simpler, which is how fast you walk when nobody is rushing you.

Researchers pooled individual data from nine separate cohort studies covering more than 34,000 community-dwelling older adults, with an average age around 73, and published the analysis in JAMA. They asked a straightforward question about whether usual walking pace predicted survival.

Every increase of 0.1 meters per second in gait speed was associated with roughly 12 percent lower mortality risk. The relationship held across all nine studies. A walking speed near 0.8 meters per second corresponded closely to the median life expectancy for a person of that age and sex, and faster speeds tracked with longer survival at every age past 65.

The finding that impressed the researchers most concerns accuracy. Predictions built from gait speed proved about as accurate as predictions built from age, sex, use of mobility aids, and self-reported function combined. One timed walk carried as much information as a small panel of other assessments.

Clinicians have since proposed calling gait speed the sixth vital sign, arguing it belongs alongside pulse, blood pressure, temperature, respiration, and pain in routine assessment of older adults. It costs nothing, needs no equipment beyond a stopwatch, and takes well under a minute.

Why Simple Beats Sophisticated Here

These measurements outperform expectations for a reason that reframes how to think about testing.

A cholesterol panel describes one system. Your fasting glucose describes another. Each gives you a precise snapshot of a single process, which is genuinely valuable and also narrow.

Grip strength and walking speed are different in kind. Both are integrated outputs, requiring your muscles, nerves, heart, lungs, joints, balance, and brain to work together in the moment. A person cannot walk briskly with a failing heart, weak legs, poor balance, or significant cognitive decline. Measuring the output measures the whole system’s current condition in a way no single blood marker can.

There is a timing advantage as well. Many blood markers stay inside their reference ranges until a problem has progressed far enough to disturb them, which is why so many people hear that everything looks normal while feeling anything but. Functional capacity starts eroding earlier and more visibly, so a declining walking speed or a weakening grip can flag a system under strain while the chemistry still reads acceptable.

A word on the limits, because this area attracts overclaiming.

The PURE researchers specifically found no significant association between grip strength and incident diabetes, hospital admission for pneumonia or lung disease, injury from falls, or fracture. Grip strength is a powerful mortality and cardiovascular predictor, and it does not predict everything, so anyone telling you a squeeze test forecasts your fall risk has gone past the evidence.

These are also population-level associations describing risk across large groups. A single below-average reading describes a trajectory to address instead of a verdict about you, and that distinction is the whole point of measuring early enough to act.

How I Look at This With Patients

Bloodwork tells me about your chemistry, and function tells me about your life, so I want both in front of me.

Adding simple functional measures to a visit changes what we can see. How much force your grip produces, how quickly you walk at your usual pace, and how easily you rise from a chair without using your arms together describe your current capacity in a way no panel does.

Tracking them over time carries even more information than any single reading. Watching your own numbers across years reveals a trajectory, and a gradual decline shows up long before it becomes something you notice in daily life.

These measures also point toward what to investigate. Declining strength alongside fatigue raises questions about thyroid function, nutrient status, hormones, and blood sugar regulation, so the functional finding becomes a prompt for the right laboratory workup instead of a replacement for it. What You Can Do Starting Now

You can measure yourself this week and start improving both numbers immediately.

  1. Time your own walking pace. Walk a measured distance at your comfortable everyday speed and convert it to meters per second. Repeat it every few months and watch the direction it moves, since your own trend carries more meaning than any published average.
  2. Test your chair rise. Stand up from a dining chair without pushing off with your hands, ideally five times in a row. Struggling with that tells you your legs need attention now, while attention still produces results.
  3. Train the systems these tests measure. Resistance work two or three times weekly builds the muscle both measurements reflect, and carrying heavy things builds grip directly through farmer’s carries, loaded walks, or simply refusing to make two trips. Hanging from a bar for as long as you comfortably can is one of the most efficient grip builders available.
  4. Add some pace to your walking. Brisk intervals train the cardiovascular capacity gait speed reflects. Support the effort with adequate protein spread across your meals and consistent sleep. Standing on one leg while you brush your teeth costs nothing and builds the balance that keeps your walking confident.

Numbers That Describe Your Life
